Liability Waiver & Health Declaration
All clients are required to complete a liability waiver and health declaration form before starting therapy. These forms help us understand your medical history, recent surgeries, and any chronic conditions to determine if lymphatic therapy is safe for you. They also ensure you are aware of possible risks, benefits, and aftercare requirements.
Clinic Policies
To provide fair and professional service for every client, please review the following policies:
Medical Clearance: Clients with certain conditions (such as congestive heart failure, uncontrolled diabetes, liver failure, or renal failure) may not be eligible for lymphatic therapy. Clearance may be required from your physician.
Cancellation Policy: We require at least 24 hours’ notice for cancellations. Missed appointments or late cancellations may result in a fee.
Late Arrivals: Arriving more than 15 minutes late may shorten your session or require rescheduling, depending on availability.
Payment Policy: Payment is due at the time of service unless otherwise arranged. We accept FSA/HSA, debit/credit cards, and online payments.
Professional Standards: All services are performed by Certified Lymphatic Therapists (CLT) and licensed professionals. Inappropriate behavior will not be tolerated and may result in immediate termination of the session.
